Individual health and wellness consultations are for people who have not yet consulted with Dr. Mackey for health-related services. Dr. Mackey will work with you to create an individualized plan of health that outlines specific steps for you to take that address your health issues, correct your health imbalances and promote healthy longevity.
There are several steps involved in creating a plan of health for you. These steps are outlined below.
Prior to Making a Telephone Consultation Appointment
• Take time to read Dr. Mackey's website to be certain her profile is aligned with your personal beliefs about healing, health and wellness care.
• You, the person with the health issue, are invited to call Dr. Mackey's office at 315-246-8559 (EST) to further inquire about her services as they relate to your health and healthcare needs.
Should Dr. Mackey not be available to speak with you at the time of your call, your call will be returned in 24 hours. Please leave your name, telephone number, nature of your call, and best time to reach you.

• Dr. Mackey will call you directly.
Be prepared to spend about 15 minutes discussing your health concerns and your questions. Dr. Mackey will ask you specific questions as well.
There is no charge to you for this call.
• Several things are necessary before making your one-hour consultation appointment.
1. After you have spoken with Dr. Mackey, you will be asked to complete requisite information forms and send them to Dr. Mackey's office. The forms include your background, health, and nutritional histories and authorization forms. Forms are sent via e-mail, fax, or post office mail (your choice) after you have spoken with Dr. Mackey.
2. Obtain the last three (3) years of your laboratory and pathology reports. Keep one copy for your records and forward a complete copy of each report to Dr. Mackey at this address: 12727 State Road 90, Locke, N.Y. 13092 (faxed copies are oftentimes unclear and illegible; faxing is not suggested for laboratory and pathology reports)
Making a One-Hour Telephone Consultation Appointment
• Once all the necessary information is obtained as outlined above, Dr. Mackey's office will call and schedule an appointment with you.
It is your responsibility to directly call Dr. Mackey's office at your scheduled appointment time.
Consultation Fees
• Full Service Telephone Consultation Total Fee: $300.00
This service includes all of the above information and the following:
→ review of your background, health and nutritional history prior to your telephone consultation appointment
→ study of your laboratory and pathology reports to look for trends underlying your immediate and other health issues, prior to your telephone consultation appointment
→ a one-hour telephone consultation with Dr. Mackey which will educate you about the findings, underlying factors contributing to your disease, and other information that you wish to discuss; the telephone conversations revolve around discussing your needs and creating a plan to meet those needs
→ the development of a specific written plan of health (client worksheet) for you that addresses your concerns and outlines particular guidelines for you to follow given your health needs; this plan provides detailed direction
→ written education reference materials mailed directly to you and used by you to review in the event you do not understand why you are doing things stated on the written plan of health/client worksheet or why you are making specific changes in your lifestyle

16-25 minutes Total Fee: $ 75.00
26-30 minutes Total Fee: $100.00
31-45 minutes Total Fee: $175.00

• Specific Herbal and/or Nutraceutical Products (only as needed)
Range: $15.00 - $300.00
Note: most herbal products and/or nutraceuticals can be obtained from your local health food store; in the event you need very specific products and cannot find them in your local area, you may purchase them from Mackey Health.
• Payment for Services
Payment is by check or money order, payable to Mackey Health, and due when you send in your completed health information and authorization forms. Service invoices are sent to you via email or by regular mail.
